Before the sun rises, Lu Qing (pseudonym) will ride her mountain bike, shuttling in the city from home to the office, a journey of more than 10 kilometers that she rides every day. She has persisted in this cycling lifestyle for over three months.
Lu Qing, 38 years old this year, has been gaining weight as she ages. In May this year, on the advice of a friend, Lu Qing chose to ride a bike to work. After enduring the initial discomfort such as sore legs caused by cycling, Lu Qing began to enjoy this low-carbon mode of transportation more and more. Three months later, she not only lost 12 pounds, but also felt more energetic.
However, starting in early August, Lu Qing constantly felt pain in her wrist joints, especially uncomfortable when gripping the handlebars. She thought it would recover on its own, but after half a month passed without improvement, the situation worsened, to the point where she couldn’t even open bottles or turn keys, let alone open the door to her house. This seriously affected Ms. Lu’s life, so she finally came to the orthopedics department of the First People’s Hospital of Hangzhou for consultation.
Unexpectedly contracted

After detailed inquiry into the medical history and ruling out recent injuries, orthopedic physician Xie Tao believed that Lu Qing’s condition was not a simple wrist sprain. Subsequently, he arranged an MRI scan for her, which revealed that Lu Qing was suffering from a triangular fibrocartilage complex (TFCC) injury in the wrist joint.
Common symptoms of TFCC injury are pain or soreness in the wrist, as well as weakness in the wrist, especially when rotating the forearm (such as wringing towels or bottle caps, driving, or turning door locks), some patients may also hear clicking sounds when moving their wrists.
This is a difficult-to-cure disease, with a long recovery period and a high propensity for recurrence, with some patients remaining unhealed for more than a decade.
In Lu Qing’s case, Dr. Xie Tao suggested that she undergo conservative treatment first, using a wrist support brace for 4-6 weeks before proceeding with rehabilitation training.

“This situation often occurs in sports enthusiasts, and because it is difficult to heal, it is often referred to as ‘immortal cancer’,” Xie Tao said. TFCC injury is generally caused by prolonged and excessive dorsal extension of the wrist joint, leading to displacement and compression of the radial head against the dorsal side, and pulling and stretching the triangular fibrocartilage. Most cases result from falling with the wrist supporting the ground, causing acute dorsal extension violence to the wrist.
If there is no traumatic violence, TFCC injury in males is more commonly associated with strength training, such as forcefully extending the wrist during dumbbell press. TFCC injury in females may not necessarily have a clear cause, some women may experience TFCC injury due to unstable development of the distal radioulnar joint, which can occur from repeated rotational pulling exercises with small weights.
In fact, Lu’s situation was caused by long-term wrist extension while gripping the handlebars.
Xie Tao further mentioned that he encountered cases related to badminton, tennis, and other sports that resulted in TFCC injuries, even push-ups could lead to TFCC injury. “This situation is quite common among sports enthusiasts, but it is often overlooked, easily confused with sprains, missing the optimal treatment window,” Xie Tao said.

But pay attention to the correct posture
During cycling, especially when using drop handlebars, choosing an inappropriate bike can exert excessive weight on the wrists, leading to excessive pressure on the wrists, which can easily cause sports injuries. In addition, some people do not ride bicycles properly, leading to excessive wear on the knees, making knees age prematurely. Therefore, choosing a suitable bicycle is crucial.
Handlebars should be appropriate
Select the appropriate handlebar width, size, and shape, for non-professional athletes, increasing the gripping area and range, reducing pressure.
Reduce vibration
Choose handlebars that provide shock protection, or wear cycling gloves, or wrap the handlebars with tape to relieve pressure.
Correct cycling posture
The correct cycling posture should involve slightly bent elbows, rather than forcefully pressing down on the handlebars, try to avoid pressing on the handlebars with force for an extended period, change the position and posture of your hands on the handlebars every 10 minutes.
